- Two regular exams

- Two Professional prophy cleanings (in absence of gum disease)

PLAN ANNUAL COST:
- Adult Plan: $400
- Child Plan: $300*
- Child plan is valid for child/ children ages 18 and under.*
- Discounted services:
15% off the normal rate for fillings, core build up, periodontal treatments, crowns, bridges, bleaching trays, root canals, night guards, complete dentures, partial dentures, implant placement, implant restorations, veneers and extractions. Excludes Invisalign. Discounts cannot be applied to any procedures being submitted to insurance.
