PLAN INCLUDES
- Two regular exams
- One emergency exam
- Two Professional prophy cleanings (in absence of gum disease)
- Two oral cancer screening exams
- All x-rays needed throughout the year
- Two Fluoride treatments for patients age 18 and under
PLAN ANNUAL COST:
- Individual Adult Plan: $400
- Child Plan* (Age 18 and Under): $300
- Family Plan:
- Two Members: $680
- Three Members: $1020
- Four Members: $ 1360
- Child plan is valid only with family members living under the same roof and children ages 18 and under.
- *Discounted services (15% of the normal rate) for fillings and core build up, periodontal treatments, crowns and bridges, cosmetic work (veneers, whitening), root canals, night guards, dentures and partials, implant restorations, and extractions. Excludes Invisalign, Implant placement. Discounts cannot be combined with Care Credit Financing. Discounts cannot be applied to any procedures being submitted to insurance.
